Patient Rights

Orchard Park Hospital and its staff support the rights of patients receiving care at all times. Patients of Orchard Park hospital have the following rights:

  1. To receive care, treatment, and services in the least restrictive environment in accordance with laws and regulations.
  2. To receive care in a safe environment free from all forms of abuse, neglect, and mistreatment.
  3. To practice religious beliefs and to be visited by their spiritual advisor at any reasonable time.
  4. To refuse care, treatment, and services unless such care, treatment, and services are necessary to
    prevent the patient from serious harm to self or others. The attending psychiatrist will inform the
    patient and/or parent/guardian of the risks of refusal. The patient shall not be subject to such
    procedures as psychosurgery, electroconvulsive therapy experimental medication, aversion
    therapy, or hazardous treatment procedures without the written consent of the patient and/or
    guardian (if applicable).
  5. To have the nature of all prescribed medications explained, the reason for the prescription, the most common side-effects, and the nature of any other treatments ordered.
  6. To confidentiality of their medical records. Information is released in a manner consistent with
    Federal and State laws. Orchard Park Hospital, LLC, shall ensure that staff members inform each
    patient and/or parent/guardian of the right to access information contained in their medical record within a reasonable time.
  7. To access advocacy and/or protective services.
  8. To be provided with a skilled interpreter or communication aide.
  9. To formulate advance directives regarding mental and physical health treatment, with
    parent/guardian, and to have those directives followed.
  10. To be free from the use of seclusion and restraints of any form that are not medically necessary.
  11. To voice opinions, recommendations, and grievances related to policies and services offered.
  12. To visitation with appropriate family members within the Hospital’s visitation guideline as indicated
    in the Patient Handbook.
  13. To receive visits from one’s attorney, physician, or clergy person at any reasonable time.
  14. To refuse visitation with any individual.

While patient rights are ensured at all times, certain privileges may be reasonably restricted by the attending physician in accordance with state law to protect the patient or others from harm, harassment, and intimidation.

  2. Ask us to correct your medical record if it is incorrect or incomplete.
  3. Request confidential communications in a specific manner most convenient to you.

  6. Request a paper copy of our Privacy Practices at any time.
  7. Choose someone to act for you as a medical power of attorney.
  8. File a complaint if you feel your rights are violated.
